FINANCIAL AND BILLING INFORMATION
SURGERY CENTER OF SOUTHWEST FLORIDA is providing this information to explain our financial and billing policies. We will provide medical services for you with the understanding that you have agreed to these terms. We appreciate your assistance in reducing the overall costs of your medical care by cooperating with these policies.
Patients and/or Legal Guardians may request from this facility a more personalized estimate of charges and other information.
Please be aware that there could be three different bills generated as a result of your surgery. This is often true, regardless of where you have your surgery performed. All are billed separately and independently of each other. The three bills are as follows:
- Surgery Center of Southwest Florida’s bill – This is for the use of the facility, staff and supplies.
- Your Surgeon’s bill – This is the surgeon’s fee for performing your surgery.
- Anesthesiology bill – This is the anesthesiologist’s fee for administering your anesthesia. Any questions regarding anesthesia can be directed to Outpatient Anesthesia of SWFL. The billing office can be reached at 239-939-2622.
- BCBS of Florida ONLY: If there is an implant used for your procedure today, you MAY receive an additional bill for your portion of that cost from IPG (Implantable Provider Group). Any questions should be directed to 866-753-1146.
- There could be additional charges if pathology and/or lab services are used in the event of a biopsy, etc. at time of surgery and/or procedure.
It is the responsibility of the patient to verify insurance benefits to determine if their plan is in-network or out- of-network. Surgery Center of Southwest Florida does accept most insurance plans, including Medicare, Aetna, Blue Cross/Blue Shield, Cigna, and United Health Care.

Your medical insurance policy is a contract between you and your insurance carrier and Surgery Center of Southwest Florida is not a party to that contract. As a result, your coverage and responsibilities are determined by your policy and you are responsible for understanding and following their required procedures. On your behalf, Surgery Center of Southwest Florida will submit all claims for our services to your primary and secondary insurance providers. It is your responsibility to provide us with sufficient, accurate and up-to-date insurance information. If your insurance company does not submit payment, you are liable for your account balance and we will request immediate payment from you. It is your responsibility to contact your insurance company with any questions and to respond to any inquiries from them in a timely manner regarding your condition or procedure. In some instances, even though Surgery Center of Southwest Florida files the claim on your behalf, your insurance company may send Surgery Center of Southwest Florida’s payment directly to you. If you receive a payment directly from the insurance company, you hereby agree to immediately forward the payment to Surgery Center of Southwest Florida.
RELEASE OF INFORMATION
Surgery Center of Southwest Florida may release your personal information to your insurer or other entity responsible for determining coverage and claim payment without any further written consent.
PRIOR AUTHORIZATIONS AND PRECERTIFICATION
If your medical insurance plan requires you to have a prior authorization or pre-certification to be on file for a procedure, this should be obtained before your scheduled appointment. IF WE DO NOT HAVE THE REQUIRED PRIOR AUTHORIZATION OR PRECERTIFICATION, YOU WILL BE FULLY RESPONSIBLE FOR PAYMENT OF SERVICES RENDERED AT SURGERY CENTER OF SOUTHWEST FLORIDA. Surgery Center of Southwest Florida makes every attempt to follow-up with your physician’s office to see that authorizations are obtained prior to the surgery; however, the authorization must be obtained by the performing physician.
AMOUNTS NOT PAID BY INSURANCE
All co-payments and deductible amounts not yet met in this calendar year and/or estimated co-insurance balances are due on the day of surgery, providing Surgery Center of Southwest Florida’s contract with your medical insurance carrier provides for collection at the time of service. Similarly, if you have no medical insurance, choose not to use your benefits, or request a service that is not covered by your insurance policy, or incur any other amount that may not be covered by insurance, we request that payment for our services be made in full at the time services are rendered, unless prior payment arrangements have been made.
After your insurance has been billed, you remain responsible for payment of the entire balance. When a balance remains we will send you a statement, which is due upon receipt. For your convenience we accept payment by Visa, MasterCard, American Express, and Discover.
Statements will be sent out monthly. Progressive collection attempts will only occur if no payments or payment plans have been received. After a third statement, if no payments or attempts to resolve the debt at this point, the services of a third party collection vendor will be engaged.
Payment plans may be arranged at the discretion of the billing department; however, adequate regular payments must be received in order to keep the account in good standing. Contact the billing office at 239-985-4600.
Care Credit is available to obtain financing option for patients who are unable to pay their medical bills in full. This financing is available for out of pocket expenses. Surgery Center of Southwest Florida will provide the patient with contact information for Care Credit so the patient may initiate the process. The financing is between the patient and the financier.
If payment plans are set up by the patient through Care Credit, the patient is responsible for the fee to set up the plan.
Patients with bad debt history in the facility must pay their old balance in full, and any anticipated current responsibility, before they can receive additional services.
PLEASE PROVIDE COMPLETE INFORMATION
If any of your billing or insurance information has changed, you need to notify our Billing Office of the updated information as soon as possible. You are responsible for keeping us informed of any insurance or address changes and failure to do so may result in your responsibility for any balance due. Surgery Center of Southwest Florida will not be responsible for any errors or lack of coverage or payment due as a result of missing or incomplete information.
